Sedalia Dental requires all new patients to fill out and sign the attached forms, and bring them with you to your first appointment. If you do not have PDF download capabilities, we will have copies available for you at the office. Please click on each form icon so you can open, print and fill out. The forms are as follows:

	Medical History Update Form Current patients must update their medical history once a year.
